If you are already a licensed healthcare professional, you may be eligible to be admitted to one of our distance education programs. Please use this checklist to get started on completing the admission requirements. We encourage you to call our Off-Site Admissions Center and speak to a Distance Enrollment Specialist who will walk you through the admission process from start to finish. Those phone numbers are provided below the following checklist.
- Application Fee – A $20 non-refundable fee is required for our distance learning programs. Please allow three business days from the time you submitted your application until you call Financial Services at 407-303-1631 to pay by credit card. Your application will not be fully processed until your non-refundable fee is received.
- Transcripts – All transcripts from colleges that you have attended are required. They must be sent directly to the Off-Site Admissions Office (2145 Metrocenter Blvd., Suite 400, Orlando, FL 32835) from the institutions attended. Hand-delivered or faxed transcripts will not be considered official. The minimum GPA for admission is 2.50.
- International Transcripts – If you completed any coursework outside of the United States, you must request a transcript to be evaluated by one of the approved foreign credential evaluations companies. You may use one of the following: AACRAO, Educational Credential Evaluators, or World Education Services.
- Other Transcripts - You may submit official transcripts for any CLEP, AP, or IB courses taken if you want them to be considered for transfer of credit.
- Copy of Licensure/Certificate - A photocopy of your professional license or credential must be faxed to our Off-Site Admissions Office. Fax numbers based on the program to which you are applying are listed below.
- Recommendation Forms – Two recommendations are required for all applicants. They must be filled out on the FHCHS form and be submitted from a professional reference (sorry, no family or friends).
