Students who do not enroll in coursework leading toward the Baccalaureate degree for a period of two trimesters must apply for reactivation to the Baccalaureate degree program by completing a FHCHS Reactivation/Readmissions form and submit current licensure. Students who do not enroll in coursework leading toward a Baccalaureate degree for more than 24 months must apply for readmission to the Baccalaureate degree program by completing a new application and submitting letters of recommendation and current licensure.
